Defects or issues identified

"There are many deficiencies re MO32, some of them are no access hatch covers, only single line safety wire/rope, safety staunchion for safety lines under one metre, many fall hazards around hatch covers with no high viz paint or covers to highlight this risk, hand holds non existent throughout. Bays 01 to 05 Lashing platforms not provided at all. Crude and dangerous attempts made to erect safety lines with wire used to lash many staunchions and safety lines in an upright position. Every Bay on the vessel has access issues to upper decks!"

  • "The vessel arrived on the 24-07-13. Attempts were made during the night shifts to unlash some bays using harnesses I'm told and some cargo had been discharged and loaded. I inspected the vessel again on the 25-07-13 at 0600hrs and sent an email to AMSA and then called their Sydney office just after 0800. An Officer arrived and I inspected the fwd areas(Bays 01 to 05). The officer said that an improvement notice would be issued to the vessel however after some inquiry have not yet been able to view it! I have notified the Sydney branch of the MUA about this. The Vessel was due to leave the next morning 25-07-13 however I have been told that a lot of material has been ordered to try and bring the vessel up to standard and would now be in port for up to five days. Work had stopped during the day on the 25-07-13 because of the obvious safety issues which was agreed to by Patrick management and the ship moved to another section of the berth in preparation for the improvement work to commence. Apparently the vessel is scheduled to visit Sydney and Brisbane only."
