"37 bay aft 11 & 12 slot has no safe platform for lashing 07 bay fwd 11 & 12 slot has no safe platform for lashing 01 bay fwd work space not 550mm. Bay is deficient, we will not load/unload any cargo from this area. 03 bay fwd, 09,17,25 bay aft has large gaps between railings need chains installed to prevent people falling overboard. Between 13/15, 21/23 & 29/31 bay, ladder access to hatch has inadequate fall protection to prevent people falling overboard. When 20' containers are loaded in 43 bay with a gap for lashing between 45 bay, workspace is less than 550mm and when loaded in the 11 & 12 slot, blocks the access to this bay."
