Defects or Issues Identified

"The main issues are fall from height risks that occur at several ares. The outboard lashing platforms do not have any provisions for portable safety railings when there is no cargo stowed FWD or AFT. and do not have any trap hatches to cover the access onto he transverse walkways. High ladders on board the vessel are unguarded (Bay 35). Rope that is used for safety railing is slack and has no means of tensioning. At Bay 01 Fwd, the sockets for the railing is corroded."

  • "The C/O and Captain was made aware in writing and seemed eager to rectify many issues. Usual practice for us is to allow first time visit vessels a chance to complete repair work before they return before we escalate any further measures."

Further Details

"Our guys did not have to work in any of the high risk areas identified as there was minimal cargo exchanged, however the situation may not be the same in other ports."
